NOTICE OF POSITION OPENING Early Intervention Service Coordinator Duties: Arrange for and participate in eligibility determination for children 0 -3, to include screening, evaluations, and ongoing assessments. Provide coordinated services to parents of infants and toddlers with disabilities or developmental delays. Develop and facilitates the Individual Family Service Plan (IFSP). Conduct family interviews, gathers information, meets with families and collaborate with the Early Intervention team. Secures reports and other pertinent information from agencies and professionals involved in delivering services. Refers and connects families to additional services and other community resources that could help strengthen the family. Develop and maintain individual case files / case notes for all referrals and eligible individuals in a confidential and up to date manner. Initiates and facilitates transition activities for children turning 3 years of age or relocating. Participate and collaborate in child find activities with local agencies and attend collaborative meetings as assigned. Fulfill the duties of Early Intervention Service Coordinators as required by the Ohio Department of Developmental Disabilities (DODD) and the Department of Children and Youth (DCY). Completes and maintains required records, reports, and documentation. Completes additional duties as assigned. Qualifications: Bachelor’s degree in audiology, child and family studies, child development, child life, counseling, education, hearing and speech sciences, human development, human ecology, human social services, medicine, nursing, nutrition science, occupational therapy, ophthalmology, optometry, physician’s assistant, physical therapy, psychology, rehabilitation, social work, or speech/language pathology. EI Service Coordinator Certification, in accordance with OAC 5123-10-04. Valid Ohio Driver’s License, acceptable driving record, and state minimum automobile insurance. Successful completion of all requested background checks, drug screens, health physical, etc. Ability to lift 50 pounds (or more to ensure health and safety of individuals served). CPR / First Aid certification; Non-violent Crisis Intervention. Workdays: Monday-Friday, 8:00 a.m. until 4:00 p.m. Weekends, evenings and holidays as required on-call. Provides early intervention services to eligible children (ages birth through three years) and their families, in everyday routines, activities, and places as developed through the IFSP. Obtains required documents for central records. Provide coordinated services to parents of children with disabilities or developmental delays. Schedules delivery of service at times and locations to meet family needs. Plans and implements instructional activities in order to assist children and families in attaining outcomes as described in their IFSPs. Initiates and facilitates transition activities for children turning 3 years of age or relocating. Assists in the preschool transition process as appropriate. Develops and maintains individual case files/case notes for all referrals and eligible individuals in a confidential and up to date manner. Participate and collaborate in child find activities with local agencies and attend collaborative meetings. Collaborates effectively with each child’s interdisciplinary team, including families, the local Help Me Grow service coordinators, therapists, preschool, Ohio Department of Developmental Disabilities (DODD) and the Department of Children and Youth (DCY), and other agencies. Implements Ohio Buckles Buckeyes child safety seat program – provides training, performs safety checks, and distributes appropriate equipment. Completes additional duties as assigned. Qualifications: Bachelor’s degree in education, health, social or behavioral science, social services, human services, or other related field. EI Service Coordinator Certification in accordance with OAC 5123-10-04 & Developmental Specialist Certification, in accordance with OAC 5123-10-05. Valid Ohio Driver’s License, acceptable driving record, and state minimum automobile insurance. Successful completion of all requested background checks, drug screens, health physical, etc. Ability to lift 50 pounds (or more to ensure health and safety of individuals served). CPR / First Aid certification; Non-violent Crisis Intervention; all requested training and meetings. The Early Intervention Specialist is expected to uphold the mission and core values of the Marion County Board of Developmental Disabilities and act in a professional, respectful manner. All agency policies, procedures, rules/regulations, and instructions should be followed throughout employment.
